Your First Visit: Therapy Evaluation
Here are some helpful tips to help prepare for your first outpatient therapy visit:
Health history: Click here to print this form and fill it out prior to your visit. Bring this completed form with you to the visit, or you will need to arrive 30 minutes early to complete when you arrive. This provides the therapist the most up-to-date information about your health when planning your care.
Insurance: It is your responsibility to verify that your insurance plan includes therapy benefits and what you are required to pay. Some insurance plans place restrictions on how much therapy you can have. We strongly recommend that you call your insurer prior to coming to therapy.
Referral: In most cases, you must have an signed order/referral (current signed within the past 60 days) from your doctor, physician assistant or nurse practitioner. This referral must contain a diagnosis and specify which therapy discipline you need (physical, occupational or speech). Please note If we have not received a referral from your doctor upon your arrival, we will may need to reschedule your evaluation.
Your first outpatient therapy visit will probably be 45-60 minutes in length.
Please bring your insurance card, valid photo ID and list of current medications to your first visit.
Pool therapy: you will not need your swimsuit on the first visit for evaluation. You will begin pool therapy on your next visit.
Wear comfortable clothing and supportive shoes. Limit jewelry and colognes/scents. Strong scents have been proven to cause respiratory distress in many patients with certain medical conditions.
Visitors: At times our facilities must restrict visitors. If you need to bring someone with you, please let us know. Children may not be left unattended, and they must remain sitting throughout your visit for their safety and for the safety of our patients.
Future appointments: Typically, you will need to return 1-2 times per week for an average of 10 total visits. Appointment times may vary, especially if you would like to see a specific therapist or be seen at a specific location or time. The staff will work with you to accommodate your needs as best they can.
