Home Health Care

Aultman’s Health Care in Your Home team consists of registered nurses and licensed practical nurses; physical, occupational and speech therapists; social workers; and home health aides who will collaborate with patients, family members and caregivers to meet each patient’s needs. Recognized by Strategic Healthcare Programs (SHP) as a “Superior Performer” for achieving an overall patient satisfaction score that ranked in the top 20% of all eligible SHP clients for the 2021 calendar year, Aultman’s Health Care in Your Home consistently provides high quality care to patients.
Aultman home health care services must be ordered by your medical provider. When your medical provider determines the type and extent of home care you should receive, your care will then be carefully planned and coordinated.
Skilled Nursing
Intermittent skilled nursing services are provided in accordance with medical provider orders. Skilled nursing care may include:
- Assessment
- Reporting of health status to medical provider
- Coordination of care provided by other disciplines
- Treatments ordered by medical provider
- Education regarding treatments, medication management and disease management
- Ostomy care and education
- Wound care and education
- Administration of intravenous fluids and medications
Therapy Services
Therapy services are provided in the home under a medical provider's direction. Therapists evaluate the patient's level of function and develop a treatment plan to assist the patient in reaching the optimal level of independence.
Physical Therapy - provided by a licensed physical therapist or physical therapist assistant under the direction of the physical therapist. Services may include: range-of-motion exercises, home exercise programs, home safety evaluations, adaptive equipment training, orthopedics, neurological disorders (dementia, stroke and Parkinson's disease), post-surgical care and gait training.
Occupational Therapy - provided by a licensed occupational therapist or occupational therapist assistant under the direction of an occupational therapist. Services may include activities of daily living training, home safety evaluation and adaptive equipment training.
Speech Therapy - provided by a certified speech therapist. Services may include speech evaluation, communication techniques, swallowing education and training, facial motor exercises and cognitive evaluation.
Home Health Aides
Our home health aides are state tested. They may assist patients and family with personal care on an intermittent basis when there is a skilled service provided in the home, in accordance with a medical provider's order. The home health aide, under the direction of a registered nurse or therapist, may provide the following services: bathing, hair care, grooming, dressing, toileting, oral hygiene and range-of-motion.
Social Services
Social services can provide patients with financial resources, community service referrals, counseling and placement assistance.
Nutritional Services
Services include education or new or uncontrolled diabetes, total parenteral nutrition (TPN), tube feedings and weight loss.
Home Infusion Services
Home Infusion services will assure the safe and accurate completion of care. Our infusion team includes pharmacists, registered nurses and support staff members who are experienced in providing in-home IV therapy services. We are committed to simplifying therapies and monitoring patients' needs to achieve treatment goals. We can promote early hospital discharge and, in some cases, serve as an alternative to hospitalization. Our services include:
- Hydration therapy
- Pain management
- Nutrition support: IV or tube feeding
- Antibiotic therapy
- Chemotherapy
- Vascular access device management, IV catheter care
- Teaching home infusion therapies to patients and caregivers
- Pharmacist on call 24/7
- Convenient home delivery
