Your First Visit

Here are some helpful tips to help prepare for your first outpatient therapy visit:

- It is your responsibility to verify that your insurance plan includes therapy benefits. Some insurance plans place restrictions on how much therapy you can have. We strongly recommend that you call your insurer prior to coming to therapy to determine if there are any restrictions.

- You must have a signed referral (current within the past 60 days) from your doctor, physician assistant or nurse practitioner. This referral must contain a diagnosis and outline which therapy discipline you need (physical, occupational or speech). Please note if we have not received a referral from your doctor upon your arrival, we will be unable to complete your evaluation.

- You will need to fill out a health history form at the start of each therapy episode. If you have been a patient in the past, this ensures that the therapist has the most up-to-date information about your health when planning your care. Click here to print and fill out the form prior to your visit. You may also fill the form out when you arrive. If you choose this option, you must arrive 20-30 minutes prior to your appointment time so that you have adequate time to complete the form. Failure to do so may result in the need to reschedule the visit.

- Your first outpatient therapy visit will probably be 45-60 minutes in length.

- Please bring your insurance card, valid photo ID and list of current medications to your first visit.

- If you are receiving pool therapy, you will not need your swimsuit on the first visit. You will begin pool therapy on your next visit.

- Wear comfortable clothing and supportive shoes. Limit jewelry and colognes/scents. Strong scents have been proven to cause respiratory distress in many patients with certain medical conditions.

- Children may not be left unattended, and they must remain sitting throughout your visit for their safety and for the safety of our patients.

- To maximize effective communication with your therapist, please refrain from using your cellphone during your visit.
