If you're referring a patient, please complete and mail or fax the physician referral form to Aultman Pain Management.

Patients
Before your first appointment, please click here to digitally sign the Aultman Hospital Therapeutic Contract. You will also need to access and complete the Pain Management Questionnaire 48 hours prior to your appointment. You may also come in to the office at least one hour prior to your appointment to fill out these forms. Failure to submit the Aultman Hospital Therapeutic Contract and the Pain Management Questionnaire will result in the cancellation of your appointment.
