*Private Pay services are not available to Medicare and Medicaid participants*

Patients must pay in advance to utilize Private Pay pricing (no adjustments or billing of insurance will be permitted after service is rendered), and payment must be made in the form of credit card or personal check (cash cannot be accepted).

To receive Private Pay, patients must request service upon registration for lab services.

To see Private Pay pricing for your lab test, please use our estimator tool. Be sure to check the list of eligible tests and note the CPT code for your test before starting.

Click our estimator tool and enter the following information:

• First Name
• Last Name
• Date of Birth
• Gender
• Email
• CPT code for your lab test (check the list of eligible tests)
• Click YES for “Are you insured?”
• Under “Insurance Name” select PRIVATE PAY.
• Under “Subscriber/Member ID” enter “1234.”
• Under “Relationship to Insured,” select I AM THE POLICY HOLDER/SUBSCRIBER
