In 2018, Aultman Inpatient Rehabilitation Facility delivered intense, medically necessary and clinically appropriate services to 476 patients on the Aultman Woodlawn Campus. All admissions must meet medical and therapy necessity criteria.  The following chart shows the types of injuries or conditions that were treated in 2018.

The majority of our patient population is 75 years and older, but our Inpatient Rehabilitation Facility also admits patients as young as 18 years old.

Clinical Excellence

Aultman Inpatient Rehab Clinical Excellence

We are focused on the clinical development of our interdisciplinary team to ensure our team is able to deliver expert and clinically proven rehabilitative care.  Many of our nurses are certified in Rehabilitation Nursing by the Association of Rehabilitation Nurses (CRRN) or Gerontological Nursing by the American Nurses Credentialing Center (RN-BC).  Several of our therapists have specialty certifications in Neuro-Developmental Treatment (NDT) or in the LOUD voice/speech treatment technique.

At Aultman Inpatient Rehabilitation Facility, we encourage our patients and families to work together with our skilled interdisciplinary care team to develop an individualized plan of clinically necessary and medically appropriate care. Our expert rehabilitation team is driven to get each patient stronger and independent as soon as possible so he or she may return to the community, which may be home, independent or assisted living settings. In 2018, the Aultman Inpatient Rehabilitation Facility treated medically complex patients, discharging more than 75.4% of our patients to the community which is comparable to the national benchmark.

Readmissions

Aultman Inpatient Rehabilitation is committed to preventing avoidable readmissions back to the hospital which adds additional costs and complications. The lower the percentage, the better. In 2018, Aultman Inpatient Rehabilitation Facility outperformed the national benchmark, successfully keeping patients from readmitting back to the hospital.

Functional Improvement

Our specially trained nurses and therapists use the Functional Independent Measure (FIM) tool to measure the change in a patient’s level of disability in motor and cognitive functioning during their rehabilitation stay. The higher the score, the better. In 2018, patients admitted to Aultman Inpatient Rehabilitation Facility had an average functional score of 62.5 for all diagnoses. At discharge, the average functional score improved to 94.8 for all diagnoses, indicating significant gains in functional independence.
