Admission

We accept referrals to the rehabilitation unit from your doctor, a social worker, an insurance company, a family member or by your request.


Admission Criteria and Common Diagnoses

Before you are admitted, our admission team will complete an assessment to determine if you meet the criteria for inpatient rehab. This assessment can be done through:

• Medical record review.
• Discussion with your physician and other caregivers.
• A meeting with you and your designated support person.

Criteria

• Be medically complex and have the ability to participate fully in and benefit from intense rehab therapy (minimum of three hours a day for five days a week).
• Require physician oversight to manage complex medical issues and guide the efforts of the team.
• Require 24/7 nursing care.
• Require two of three therapies: physical, occupational and speech.
• Have the potential to return to a community-based setting.
• Once you are admitted to the rehab unit, your rehab team will monitor your progress with the program and the goals on a weekly basis.

Common Diagnoses

• Neurological disorders.
• Brain injury.
• Hip fracture.
• Amputation.
• Stroke.
• Burns.
• Major multiple trauma.
• Congenital deformity.
• Incomplete spinal cord injury.
• Severe or advanced osteoarthritis.
• Active, system vasculidities with joint inflammation.
• Knee or hip replacement.
• Active, polyarticular rheumatoid arthritis, psoriatic arthritis and seronegative arthropathies.
• Other diagnoses may qualify for admission based on additional qualifying criteria.

Every referral we receive is given consideration for admission. Each case is reviewed on an individual basis. If you have questions about whether your diagnosis would qualify for admission, please talk to your doctor, call the admission office Monday-Friday at 330-479-3088 or fax 330-954-0581.

What to Expect Upon Admission

Your team will create a care plan to help you meet your goals. Your goals and needs will be determined through evaluations from our team members and with your input. Your team will consist of a specialty rehab physician, rehab-trained nurses and therapists, dietitians, social workers, your identified support system and other types of health care specialists deemed necessary for your care. You and your support system will receive education, training, resource and support information you can use even after you are discharged.
