• Your name
• Address
• Phone number
• Physician's name
• When and where the equipment is to be delivered
• Insurance information, if applicable

Step 3: Aultman Home Medical Supply will schedule the delivery of your equipment, contact your physician for written orders and coordinate care with other health care professionals such as nurses, hospital discharge planners and ambulances.
