Aultman OneChart is a personalized and confidential patient portal that allows you to access your health information - anytime, anywhere. 

This convenient web-based program provides a view into your electronic medical record (EMR) to promote communication with your health care providers. 

If you have any questions or need an expired invitation email resent, please send an email to patientportal@aultman.com. 

Notice: 

As a result of recent changes in Federal laws, effective August 1, 2022, all proxy accounts for patients age 13-17 will have limited access to a patient’s medical records.

Proxy users will only be able to send and receive messages with a patient’s providers and submit medication renewal requests. Patients age 13-17 with their own portal account will have full access to their medical records.

Proxy requests for copies of highly sensitive medical records for patients age 13-17 require authorization from the patient. Requests can be made by contacting Aultman Medical Records at 330-363-4244 or 330-363-3285, or at releaseofinformation@aultman.com.
