Insurance & Billing FAQs
Yes. You will need to provide us with complete primary and secondary insurance information. As a courtesy to our patients, Aultman Hospital submits bills to your insurance company. There may be times you will need to contact your insurance company and/or provide additional information in order to process your claim.
Your status is determined by your admitting physician. Sometimes, "observations" is the appropriate status used, which is considered an outpatient visit.
The following may apply:
1. Your plan may not cover the service you received.
2. You did not provide accurate insurance information at the time of your service.
3. You were not covered by your plan at the time of your service.
4. Additional information was requested from the insured before payment will be processed.
Contact your insurance company to discuss your situation.
Patient Financial Services bills for our facility and selected physicians. You may also receive a bill from a physician who treated you. If you have questions, call the patient information number on your statement.
The following may apply:
1. The statement you received may have been mailed before the payment was received and posted to your account. To verify your bill was paid, please call a customer service representative and provide your name and account number. If payment was recently mailed, please allow 7-10 days for payment to reflect on your account.
2. The statement you received may be for recurring services you receive at the hospital. Please contact a customer service representative with any questions.
If you enter on the Sixth Street side, turn left at that hallway. Patient Financial Services will be down the hall on the right. If you enter on the Seventh Street side, continue past the Information Desk. At the next hallway by the florist shop, turn right. Patient Financial Services is down the hall on the right.
Payment is expected upon receipt of the first statement, unless arrangements have been made. Please contact a Patient Financial Services customer service representative to arrange a payment plan.
Health Maintenance Organizations (HMOs) require patients to select a primary-care physician to handle their health-care needs. In order for services to be covered, patients must use the HMO's selected network of hospitals, doctors and other medical professionals. If you do not use the selected network of hospitals, doctors and other medical professionals, services will not be covered.
Preferred Provider Organizations (PPOs) provide health care through a network of hospitals, doctors and other medical professionals. Patients receive a higher benefit and pay less money if they use health-care providers within the network. If you prefer to use out-of-network hospitals, doctors and other medical professionals, some services may be covered. If you are in a PPO, and Aultman Hospital is an out-of-network provider, you may be able to use Aultman facilities. Please call 330-363-7444 to see if you qualify.
The birthday rule is used to determine primary and secondary coverage for children. The birthday rule refers only to the month and day in a calendar year, not the year the person was born. The insurance of the parent whose birthday occurs first in the calendar year is considered the primary insurance. The other parent's benefits are considered secondary insurance.
Financial counselors are available to advise you on the various assistance programs. As well, they will review your financial status to determine your eligibility for Healthcare Assurance Plan (HCAP). Under Ohio law, you may be eligible to receive basic, medically necessary hospital services without charge if you are an Ohio resident on General Assistance (GA), Disability Assistance (DA), or your income is at or below the current Federal Poverty Guidelines. This program applies only to hospital services. It does not apply to doctors' services. If you think you may qualify for this program, please contact our HCAP Financial Counselors at hcap@aultman.com.

1. A Current Procedural Terminology (CPT) Code, which can be obtained from your physician
2. The name of the procedure
3. Your name and correct mailing address
