Printable Medication Record

Medications can play an important role in helping keep us well. Medicines can be a vital part of treatment for serious infections or chronic conditions. They help relieve pain, lift depression, and can even combat some of the nation's leading causes of death and disability.

Taking your medication as directed is extremely important. It is also crucial that all of your physicians know what medications you are taking in order to prevent drug interactions. Using a medication list is an easy way to keep track of this information and to help keep you safe! Knowing your medications helps prevent common errors such as omitting a needed medicine, getting the wrong dose of medication or being prescribed a medicine to which you are allergic.

You can print out a medication record to fill out and hang up at home! Be sure to include any over-the-counter medications, vitamins and supplements. Also, make sure to keep this list updated every time there is a change in your medications.

Please remember to bring your medication record with you to the emergency room, the hospital, your pharmacy, your physician(s) and/or other health care providers.
