Aultman has partnered with Trellis Rx, a leading technology-enabled specialty pharmacy service provider, to launch Aultman Specialty Pharmacy Services. New this fall, the service line will begin offering chronic disease management and specialty pharmacy services to support patients who require complex or high-cost therapies to manage their conditions.

Because these therapies can be difficult for patients to afford and manage, patients often struggle to adhere to their therapy regimens. The specialty pharmacy will help improve adherence by embedding pharmacists and liaisons directly into Aultman ’s clinics to provide high-touch support to these patients.

Aultman Specialty Pharmacy Services embeds highly-trained pharmacists and pharmacy liaisons directly into specialty clinics to provide high-touch support to patients. Patients will have access to highly trained pharmacy liaisons to help them navigate the often-difficult process of affording and obtaining their medications. Like the clinical pharmacists, pharmacy liaisons will work on-site at Aultman to create a coordinated, seamless experience for patients and providers.

Pharmacists assist other providers by managing:
• Patient education
• Medication reconciliation
• Medication therapy management

Pharmacy liaisons handle tasks including:
• Benefits investigation
• Prior authorization
• Financial assistance
• Refill coordination
• Medication delivery
• And more

Initially, Aultman is offering these services at its Aultman Carrollton Family Medicine and Aultman Hematology & Oncology Centers for patients living with diabetes and cancer and plans to expand services over the next year to support additional patients.
